By signing up for a breathwork session with Noelle Mietus I agree to the following terms: I acknowledge that breathwork can be a transformational experience. I engage in this experience willingly and take full responsibility for my own physical, mental, and emotional experiences during and after the session. I acknowledge that this work is not appropriate for pregnant women, for persons with cardiovascular complications including angina, heart attack, high blood pressure, glaucoma, retinal detachment, osteoporosis, history of seizures, stroke, major psychiatric conditions, recent surgery, acute infectious illness, epilepsy, or other condition for which my doctor has not cleared me to practice breathwork. If you have any doubt about whether you should participate, please consult with your physician(s). Persons with asthma should bring their inhalers and consult with their physician(s) before the session. I have chosen to participate in this breathwork and my choice is voluntary. I understand that I may terminate my session at any time, and that termination of my session does not guarantee a refund if I have made payment for the session. I understand that there is no assurance that I will feel any differently after a breathwork session. I understand the potential risks of participation and hereby agree to release and hold Noelle Mietus harmless from any and all legal responsibility or liability for outcomes experienced as a result of breathwork. I agree to fully release and hold Noelle Mietus harmless from and against any and all claims, liability, or damages arising out of or in connection with my session. By submitting my name and date below, I hereby confirm that I have read and understand the above information and attest that my general health is good to participate.
